These forms are only for RSVP volunteers who have been onboarded and given instructions to use these materials. First-time payees must fill out the ACH form (for direct deposit) when they submit their RSVP Service Report. Subsequent monthly submissions only require the RSVP Service Report as long as banking information does not change.
