How to get help

Call our options counselor to discuss your options at 801-229-3804 or email aging@magutah.gov.

After assessing your needs and completing a screening over the phone, you will be placed on the appropriate waiting list.

Once you are accepted into a program, you will be assigned a case manager who acts as your advocate assisting in developing a care plan to support you through the aging process. Your case manager meets with you regularly to facilitate access to non-medical services to meet your needs.
